Family Community Case Management provides case management support for youth during times of adjudication.
We serve at-risk youth and, if necessary, their immediate family. Case management links the at-risk youth to needed community services. Case management for juveniles involves a list of services (including state services) to help juveniles and their families while they are in the Family Community Case Management system on an as needed basis.
