Family Community Case Management Services range from individual care to family evaluation to better help both at-risk youth and their families.
Individual and Family Therapy
Administered by a licensed therapist, therapy services are available to help bring structure to the family unit for a peaceful coexistence.
Case Management
Family Community Case Management will provide support for youth and their families during times of adjudication and link clients to community services.
Youth Advocacy
Family Community Case Management provides part-time legal support, mentoring, and case management for at-risk youth during times of adjudication. This service assists in the youth’s compliance with court-ordered sanctions and/or directives decreasing the probability of criminal re-entry.
Youth Mentoring Services
Family Community Case Management mentors youth, providing emotional and academic support to assist with academic success.
Adult Peer Support Services
Peer support is a system of giving and receiving help founded on the key principles of respect, shared responsibility, and mutual agreement of what is helpful. Family Community Case Management provides Certified Mental Health Peer Specialists to help our adult clients find affiliation and connection without the constraints of traditional (expert/patient) relationships.
