Family Community Case Management provides support to individuals with emotional, mental, or physical experiences that effect their daily life. We provided a
peer-led wellness recovery by focusing on goal setting, mentoring, problem solving, skills training, and community resources.

Peer-Led Wellness and Recovery
Peer support is a voluntary process of giving and receiving encouragement and assistance to achieve long-term recovery. Utilizing individuals who share a unique experience in their journey of healing and well-being, Peer support offers:
- Emotional support
- Shared knowledge
- Practical assistance
- Connection people with resources, opportunities, and communities of support
